Miniwheelers Football Camp

Mini-Wheelers Football Camp

Come work with the Pros! Join your favorite Steamwheelers players and coaches as they teach you vital football skills while having FUN!

Camp Details:
Date: June 26th
Time: 1:00 PM – 3:00 PM (Registration begins at 12:00 PM)
Cost: $65 per camper
Ages: 4 – 15

Each participant will receive:

  • One FREE ticket to the June 28th game
  • An official Mini-wheeler camp T-shirt

Important:
To guarantee your child receives a camp T-shirt (and correct size), registration must be completed by June 15th. Registrations submitted after this date may not receive a shirt or preferred size.

The cost of the camp is $65 per camper.

Ticket Range *
Please select the ticket range you would like to sit in. Your mini-wheelers tickets are in the $15 ticket value. If you are wanting to purchase a diferent price range you will need to pay the difference of your childs ticket.

Please indicate how many additional tickets you would like to purchase. Each camper will receive one FREE ticket for the June 28th game (for the mini-wheelers admission), and any additional tickets for family and friends must be purchased.
This ticket amount will need to include the camper ticket(s)

Please include total ticket price and football camp price.
Please submit your 3 digit code on the back of your card.

WAIVER/RELEASE FORM - MINI WHEELERS CAMP 

 

I. PARENTAL CONSENT

 

I, The parent or legal guardian of (Child Name) ____________________________________________, a participant in the Steamwheelers football camp, does hereby grant permission for his/her participation in any and all camp activities.

 

II. RELEASE FROM LIABILITY

I agree to assume all risks and hazards incidental to participation in a camp activity.  I do hereby waive, release, absolve, indemnify, and agree to hold harmless Steamwheelers Football, front office, coaches, sponsors, players, participants, and persons for any claim arising out of an injury to my child, whether the result of negligence or any other cause.

 

III. MEDICAL RELEASE

Because your child is involved in an active camp, there may be an occasion when an injury occurs that requires medical treatment and we are unable to contact you.  This situation may occur before, during or after our camp while at our site.

 

IV. IMAGE  RELEASE

I hereby grant the Quad City Steamwheelers and its agents permission to use photographic portraits, pictures, digital images or videotapes of me, or in which I may be included in whole or part,  or reproductions thereof in color or otherwise for any lawful purpose whatsoever,  including  but not limited to use in any Team publications or team websites, without payment or any other consideration. I hereby waive any right that I may have to inspect and/or approve the finished  product or the copy that may be used in connection therewith, wherein my likeness appears, or the use to which it may be applied.
